Use this checklist to gather the information most often needed when arranging veteran funeral services, military funeral honors, and VA burial benefit guidance. VA benefit rates can change, so verify current rates at va.gov before relying on any amount.
Military and identity documents

- ☐Legal name, date of birth, Social Security number, and branch of service
- ☐Service dates, discharge status, and rank if known
- ☐Next-of-kin contact information
Family decisions to discuss
- ☐Private cemetery, Utah Veterans Cemetery in Bluffdale, Fort Douglas, cremation placement, or another option
- ☐Whether the family wants military funeral honors, flag presentation, Taps, clergy, or other ceremonial elements
- ☐Preferred service location, music, readings, photos, obituary details, and military memorabilia to display
- ☐Who will be the main family contact for documents and approvals
Benefit and cemetery questions
- ☐Review current VA burial allowance rates directly at va.gov before making financial plans
- ☐Confirm cemetery eligibility, scheduling, committal options, and dependent burial rules
